If you are interested in becoming an active agent with our Company, please submit the following information:

• Completed Producer Questionnaire
• Signed Producer Agreement †
• Marketing Consent Form
• Minimum Premium Commitment Form
  (The above forms are available for download by clicking on the link below) 
• A copy of the declaration page from your agency Errors & Omissions policy
• P&C license for each individual contacting us for quotes* 
• Broker’s license (Required for NC & SC)
• Agency’s state license (when applicable)

† Must have witness signature(s); Completely sign and initial all sections with an asterisk (*).

* Please note agent’s Social Security Number and/or license number on each license.

The information will be reviewed by our Management Review Team. You will receive an email with their decision. If your agency is approved, we will sign the Producer Agreement and return the signed original copy to you for your files. Contact the Marketing Department with any questions.
